4、經(jīng)衛(wèi)生部認(rèn)定的檢驗(yàn)機(jī)構(gòu)出具的檢驗(yàn)報(bào)告及相關(guān)資料，按下列順序排列：A. 檢驗(yàn)申請(qǐng);B. 檢驗(yàn)受理通知書;C. 產(chǎn)品說明書;D. 衛(wèi)生學(xué)(微生物、理化)檢驗(yàn)報(bào)告;D. 毒理學(xué)安全性檢驗(yàn)報(bào)告；
5、產(chǎn)品原包裝(含產(chǎn)品標(biāo)簽)。擬專為中國(guó)市場(chǎng)設(shè)計(jì)包裝上市的，需同時(shí)提供產(chǎn)品設(shè)計(jì)包裝(含產(chǎn)品標(biāo)簽);
6、產(chǎn)品在生產(chǎn)國(guó)(地區(qū))或原產(chǎn)國(guó)(地區(qū))允許生產(chǎn)銷售的Z明文件;
7、進(jìn)口化妝品報(bào)關(guān)需提供其中文標(biāo)簽、貨物入境檢驗(yàn)檢疫通關(guān)單、產(chǎn)品成分表、contract、invoice、裝箱單和報(bào)檢、報(bào)關(guān)委托書等手續(xù)。

3. Product quality standard;
4. The inspection report and relevant data issued by the inspection institution recognized by the Ministry of health are listed in the following order: A. inspection application; B. inspection acceptance notice; C. product specification; D. hygiene (microorganism, physical and chemical) inspection report; D. toxicology safety inspection report;
5. Original packaging (including product label). Product design packaging (including product label) shall be provided at the same time if it is planned to be specially designed for the Chinese market;
6. Z-document that the product is allowed to be produced and sold in the country (region) of production or the country (region) of origin;
7. For customs declaration of imported cosmetics, the following procedures shall be provided: Chinese label, goods entry inspection and quarantine customs bance b, product comb list, contract, invoice, packing list, inspection declaration, customs declaration power of attorney, etc.
